When decay reaches the nerve inside a tooth, the nerve becomes infected. That is the pain that keeps you awake, throbs when you lie down, and does not respond to painkillers for long. At that stage there are two options: remove the tooth, or remove the nerve and keep the tooth. A root canal is the second.
The infected tissue is cleaned out of the canals inside the root, the space is disinfected and sealed, and the tooth is then capped with a crown so it does not crack. The tooth stays in your mouth and works normally.
Root canals have an unfair reputation for being painful. The pain people remember is the infection that brought them in, not the treatment — the treatment is what stops it. Done under proper anaesthetic, an RCT feels much like having a filling done.
The alternative to a root canal is losing the tooth. That is the actual choice, and it is worth being clear about: a tooth extraction is quicker and cheaper on the day, and then you are deciding how to fill the gap for the next thirty years. An X-ray at the first visit usually shows which option the tooth still allows.

The tooth is numbed and a small opening made in the top of it.

The infected tissue is removed, the canals shaped and disinfected.

Sealing
The canals are filled with an inert material so bacteria cannot recolonise them.

Crown
A root-treated tooth is brittle and needs a crown. Skipping this is the usual reason an RCT tooth fails later.

Is a root canal painful?
The treatment is done under local anaesthetic and should not hurt. The infection beforehand is what hurts. Most patients say afterwards that they had expected far worse.
Do I really need a crown after the root canal?
For back teeth, yes. A root-treated tooth has lost its blood supply and becomes brittle, and chewing forces crack it. A cracked root-treated tooth usually cannot be saved a second time — so skipping the crown risks losing the tooth you just paid to save.
Can the tooth be extracted instead? It is cheaper.
It is cheaper today. But a gap lets the neighbouring teeth tilt and the opposing tooth drift down, and replacing the tooth later with an implant or bridge costs several times what the root canal and crown would have. We will tell you honestly when a tooth is genuinely not worth saving.
How many visits will it take?
Often a single long visit for a straightforward front tooth; two or three for a molar with curved canals or an active infection that needs settling first.
