When a tooth has been missing for years, the bone that held it gradually shrinks — there is nothing loading it any more. By the time someone decides on an implant, there is sometimes not enough bone left to hold one.
Grafting rebuilds the site. Graft material is placed where bone is missing and your own bone grows through and replaces it over several months. In the upper back jaw, where the sinus sits directly above the missing molars, a sinus lift raises the sinus floor to create the height an implant needs.
This is why we X-ray before quoting for an implant. Being told you need a graft is not a sales tactic — it is the difference between an implant that holds and one that fails in two years.
What bone grafting involves

X-ray or CT to measure exactly how much bone is present and how much is needed.

Grafting
Graft material is placed and the site closed. Done under local anaesthetic.

Healing
Four to nine months for your own bone to replace the graft, depending on the size of the site.

Once the bone has matured, the implant is placed as normal.

Does the graft come from my own body?
Usually not. Most grafting uses processed graft material, which avoids a second surgical site. For larger reconstructions your own bone may be used, and we would discuss that fully first.
Can I avoid the graft?
Sometimes — shorter or angled implants, or a different implant position, can occasionally work around a shortage of bone. We will look at that before recommending a graft, because it saves you both time and money when it is possible.
How much extra time does this add?
Four to nine months before the implant can go in. It is the main reason an implant plan stretches over a year, and it is worth knowing at the outset rather than mid-treatment.
