A tooth gem is a small crystal fixed to the front surface of a tooth with the same bonding material used for a filling. There is no drilling and nothing is removed from the tooth. It comes off cleanly whenever you decide, and the tooth underneath is untouched.
The reason to have it done at a dental clinic rather than a salon is the bond. Done properly the edges are sealed and smooth, so plaque cannot collect beneath it. Done badly, a gem traps bacteria against the enamel and leaves a patch of decay when it eventually falls off.

Choosing
Pick the crystal and the tooth — usually an upper canine or lateral incisor.

Preparation
The surface is cleaned and conditioned. Nothing is drilled.

Bonding
The gem is bonded and cured. Around 15 minutes in total.

Brush normally. Avoid biting hard on that tooth for 24 hours.

How long does it stay on?
Typically six months to a couple of years. It can come off earlier if knocked, and can be re-bonded.
